To be considered an official member of FOHR, you need to give us a signed copy of the Assumption of Risk form. In addition, to enter the H.O.R.S.E. Rescue premises, we must have a signed copy of the H.O.R.S.E. Rescue Waiver and Volunteer Application (for weekly volunteers only).
