Immunization and Record Request

If you are a current student or recently graduated (< 3 months ago) you may view and print a copy of your immunizations.                                             

•      Login to the student portal at https://myhealth.geneseo.edu
  
•      Click on IMMUNIZATIONS and you may print off a copy

If you are a current student and would like anything other than your immunization records you must fill out the Request for Immunizations or Medical Records form.

If you are not a current student and would like your immunization records or complete medical/treatment records please download the Request for Immunizations or Medical Records form.

There is not a cost for requesting your immunization records.  There is a cost of $8.00 for the release of your complete medical/treatment records.  Payment is required in the form of either 1) a check made out to SUNY Geneseo or 2) credit card.

Please mail your request to Lauderdale Health Center, 1 College Circle, SUNY Geneseo, Geneseo, NY, 14454 OR fax your request to 585-245-5744 (credit card payments only--see above).  Please allow for a turn-around time of 3-5 business days.
