Step 2

Once you complete your enrollment, print the confirmation page and return this to Human Resources.

Step 3
(complete if you have prior membership with Employees' Retirement System (ERS)

Complete  Employees' Retirement System Deterimination Form (ORP-4).  Return completed and signed form to Human Resources, Doty Room 318.
