SUNY-OIP Long Application Form:

The SUNY-OIP application for semester/academic year programs is eight pages long and consists of the following:

  • Coverpage
  • Application Form
  • Study Statement
  • Lanuage Proficiency Form
  • Confidential Academic Reference Forms (2)
  • Judicial Review Authorization

Coverpage: Page 1 is a cover-page with an overview of the program name, application deadlines, and a checklist outlining all the items you will need to submit the application.

Application Form: Page 2 - 3 is the application form itself.  These two pages are fairly straightforward: please make sure to use your Geneseo P.O. box for the 'Local Address" and your @geneseo.edu webmail address (for students applying from other SUNY schools, please use your SUNY campus address and SUNY email).

Study Statement: Page 4 is your statement of intent for studying abroad.  It should be a concise (about 1 - 2 paragraphs) statement outlining how your study abroad relates to your academic achievements and goals, how you plan on integrating your study abroad experience into your academic program, etc.  This page must be signed by your academic adviser: this will give you a chance to review your statement, ideas, and goals with your adviser.  While handwritten statements will be accepted, a typewritten and attached statement is always a good idea.

Language Proficiency Form: Page 5 is an assessment of your foreign language proficiency level and only needs to be filled out if you are applying to a program in which instruction/coursework are in a language other than English.  If this description fits the program you are applying for, have one of your foreign language professors fill out the bottom half of this form.

Confidential References: Page 6 - 7 are the academic reference forms.  We higly recommed that you request actual reference letters from your professor in addition to having the form filled out: in the likely event that the international university you are applying to requests letters of recommendation, SUNY Geneseo Study Abroad can simply make copies of the letters you submit to us and forward the originals (as opposed to you having to go back to your professor to request another reference).

Judicial Review Authorization: Page 8 - 9 authorize SUNY Geneseo Study Abroad to review your judicial standing at your home institution.  SUNY Geneseo students only need to complete page 8 (the first page of the judicial review form).  Students from other SUNY institutions skip page 8 and complete page 9; please have a judicial officer at your home institution sign the page before you send it to Geneseo.
