Registration and Application Forms:
Are you ready to go to Biloxi, Mississippi on one of our four upcoming trips? You'll need to follow these three easy steps to complete your registration. PLEASE READ AND FOLLOW THE INSTRUCTIONS BELOW CAREFULLY BEFORE FILLING OUT THE FORMS TO AVOID PROBLEMS AND HASSLES:
- Complete all 8 forms per individual. They are listed in the chart of Individual Registration Forms below.
Each agency with whom we work requires their own set of paperwork, and we appreciate your dilligence in completing all forms.
- NOTE: The forms are provided in two formats: MS Word and PDF.
You need to turn in 1 completed copy of each form. DO NOT print out all 16.
- MS Word documents may be opened and completed by typing on your computer before printing them out for easier legibility.
- PDF files are for those who do not have MS Word on their computer. These need to be printed off first and filled in by hand.
- Each hand printed form MUST be filled out completely and legibly in blue or black ink, only
- Be sure you wait to sign #2. Livingston CARES Release and Waiver of Liability Form***. This must be done in the presence of a notary public. There is a notary public available at the Federal Credit Union in MacVittie College Union. Be sure to check their hours on their website before making a special trip.
- Bring the completed forms and your check for the trip fees to Livingston CARES in the MacVittie College Union Center for Community Room 353. A staff member will review it and confirm your seat or place you on a wait list if all seats for a specific trip are filled.
