To be considered an official member of FoHR, you need to give us a signed copy of the Assumption of Risk form. In addition, to enter the H.O.R.S.E. Rescue premises, the Rescue must have a signed copy of the H.O.R.S.E. Rescue Waiver and Volunteer Application (VA for weekly volunteers only). Links to download these forms are below.
