Armory Permission Waiver (PDF): This form serves as a manifest. Complete this form to document equipment stored in the Armory.
Armory Overnight Form: Complete this form if you plan on using equipment from the Armory over night.

Change Officer Form: Complete when there are any changes made to an organization's executive board.

Club Sports Waiver: Completed by student who understands the risks associated with club sports and releases the school from liability regarding any injuries sustained during the participation in a club sport.
