Case Management
Case Management aims to provide short-term and goal-oriented interventions for students with concerns in singular or multiple life areas. Case Managers directly support students in overcoming obstacles, reducing distress, and connecting to appropriate on- and off-campus resources. Case Management provides ongoing non-clinical support to students referred to the Geneseo CAREs Team and collaborates with other CAREs Team members on campus safety and risk assessment.
How to Connect with a Case Manager
Students can connect with Case Management support in several ways:
- Use the Geneseo CAREs Referral Form.
- Email Rachel Radovich-Chu, Geneseo CAREs Coordinator.
- Discuss your needs with a campus community member (i.e. academic advisor, professor, counselor, coach, residence life staff member), who can submit a referral on your behalf.
What to Expect:
Upon receiving a referral, a Case Manager will reach out to schedule via email. Depending on your circumstances, you may have one or multiple appointments throughout the semester; however all students will have an initial intake appointment to assess areas of wellness and need for resources.
Case Managers provide services such as initial assessment of needs, follow-up appointments, referral coordination, consultation and collaboration with other on campus and off-campus resources.
Below are some examples of case management assistance provided to students:
- Researching and referring students to health and wellness resources
- Coordinating student care services with local hospitals during health and safety emergencies
- Navigating social services and community agency networks (i.e. Livingston County, LawNY, NYS of Health/Medicaid, etc.)
- Assisting students through college transitions (e.g. new student experience, leave of medical leave of absence, transfer/withdrawal, etc.)
- Advocating with students for additional academic resources and support
- Connecting with Financial Aid to organize and complete required paperwork
Services not within the scope of Case Management include the following:
- Ongoing therapy/mental health counseling treatment
- Diagnostic services
- Documentation of a disorder, disability, or need for accommodations
- Therapy animal/emotional support animal documentation
