Health Requirements - New International and Exchange Students

Welcome new international and exchange students! This page will assist you in completing your health requirements.

Please begin by reviewing Next Steps for Admitted International Students, which contains a checklist of required action items.

Refer to the following items on the Next Steps page:

  • Item #4, "Claim your myGeneseo account"
    This step is required before you can complete your health requirements. You will receive instructions for this process once you have been assigned your Geneseo email address.
  • Item #8, "Complete health and immunization requirements before arrival"
    This refers to your Health Entrance Requirements. These requirements are due before you arrive on campus. Details are provided below.

Once you have claimed your myGeneseo account, please review Health Requirements - New Incoming Students. These are requirements for ALL incoming students that you will need to complete. This page contains several online "forms" as well as true records that are required to be submitted by you.

You can complete the online forms listed below without a physician/medical provider assistance. 

  1. Health history (about your health, medications, allergies)
  2. Tuberculosis (TB) screening questions
  3. HIV Testing offer
  4. Insurance (DO NOT complete this part. You will not have this information until after you arrive.)

All true records listed below must be submitted in English and signed by a medical professional. This includes your physical evaluation and immunization records.

True records to submit:

  1. Physical Evaluation
  2. Immunization Record
  3. Consent for Medical Treatment of a Minor 
    (complete this only if you will be under 18 years old when you arrive to campus)
